Stir together water and coffee concentrate. Combine the resulting coffee with the hazelnut-chocolate spread in a blender and process them until they become a single, smooth mixture. Pour the coffee mixture over the coffee ice cubes in a tall glass, and top with half and half (or don't if you like your coffee a little darker).
